Therapie periimplantärer entzündlicher Läsionen

Nichtchirurgische und chirurgisch-rekonstruktive Ansätze

Zusammenfassung

Hintergrund

Die beeindruckende Erfolgsgeschichte der dentalen Implantologie mit oft signifikantem Gewinn an Lebensqualität für die Patienten kann in Zukunft nur fortgeschrieben werden, wenn sich Kliniker und Grundlagenwissenschaftler aktiv dem Problem der periimplantären Infektionen annehmen. Ziel des vorliegenden Beitrags ist, die Multikomplexität periimplantärer Probleme aufzuzeigen, das mechanobiologische Verständnis zu fördern und davon ausgehend kritisch die Indikationsstellung rekonstruktiver Maßnahmen unter dem Aspekt der Nachhaltigkeit abzuleiten.

Methoden

Im Rahmen eines kurzen Literaturüberblicks werden Ursachen und Häufigkeit periimplantärer Infektionen dargestellt und die derzeit gängigen Methoden der Taschenelimination präsentiert. Dabei wird auf die aktuellen Stellungnahmen der wissenschaftlichen Gesellschaften in Deutschland Bezug genommen. Unter mechanobiologischen Gesichtspunkten werden periimplantäre Defekte analysiert und die Bedingungen formuliert, unter denen eine hinreichende Regeneration erwartet werden kann.

Schlussfolgerungen

Eine nachhaltige und individuelle Therapiestrategie periimplantärer Infektionen kann nur entwickelt werden, wenn individuelle medizinisch-systemische, konstitutionelle sowie lokale Wirtsfaktoren und deren Reaktion auf komplexe mechanische Strukturen analysiert und auch verstanden werden. Unter diesem Aspekt eröffnen sich gerade für das Fachgebiet der Mund-, Kiefer- und Gesichtschirurgie zahlreiche Möglichkeiten und Chancen, die Diskussion um die Prävention und Behandlung periimplantärer Infektionen aktiv zum Wohle der Patienten zu gestalten und Visionen für zukünftige Forschungen zu entwickeln.
