Zerebrale Ischämie nach Bestrahlungen im Kopf-/Halsbereich

Zusammenfassung

Hintergrund

Nach Bestrahlungen im Kopf-/Halsbereich kommt es zu einer Vaskulopathie mit vermehrtem Auftreten radiogener Stenosen der hirnzuführenden Gefäße. Konsekutiv zeigt sich eine Erhöhung des Risikos für zerebrovaskuläre Ereignisse (Schlaganfälle oder transiente ischämische Attacken). Es soll eine kurze Darstellung der vaskulären Veränderungen und eine aktuelle Übersicht zu Epidemiologie, Pathogenese und Behandlung erfolgen.

Material und Methoden

Grundlage bildet eine Suche in PubMed mit den Begriffen „radiation“ und „stroke“. Ausgewertet wurden 1468 Ergebnisse und relevante Literatur. Ausgeschlossen wurden Fallberichte und Patienten mit Moyamoya-Syndrom. Priorisiert wurden prospektive kontrollierte Studien und größere retrospektive Studien mit Bestrahlung der großen Gefäße im Kopf-/Halsbereich. Insgesamt konnten 19 Studien in die Auswertung einbezogen werden.

Ergebnis

Pathophysiologisch kommt es zu einer Läsion des Intima-media-Bereichs und der Adventitia. Arteriosklerotische Veränderungen treten unabhängig von bestehenden vaskulären Risikofaktoren auf. Das Risiko von Arteriosklerose und zerebralen ischämischen Ereignissen nach Bestrahlung des Kopf-/Halsbereichs ist mindestens verdoppelt. Es besteht eine Risikoerhöhung gegenüber der Normalpopulation und gegenüber Tumorpatienten desselben Kollektivs, die mit Chemotherapie und/oder operativ behandelt wurden. Bei unilateralem Bestrahlungsfeld zeigt sich im Seitenvergleich ipsilateral eine Erhöhung des Risikos für vaskuläre Ereignisse. Bestrahlungen im Brustbereich führen zu einer geringen Risikoerhöhung hinsichtlich zerebrovaskulärer Ereignisse. Das Auftreten radiogener Stenosen korreliert mit der Überlebenszeit nach Bestrahlung. Die Angaben zur Schlaganfallinzidenz reichen von 4 % bis 19 % in 5 Jahren. Die Daten zur Abhängigkeit der Bestrahlungsdosis sind uneinheitlich, einige Studien legen aber vor allem für höhere Dosierungen einen Zusammenhang nahe.

Schlussfolgerung

Bestrahlungen im Kopf-/Halsbereich führen zu einer Vaskulopathie und erhöhen damit, häufig bereits auch in jungen Jahren, signifikant das Risiko transienter ischämischer Attacken und des ischämischen Schlaganfalls mit einer Latenz nach Bestrahlung von Monaten bis zu Jahrzehnten. Daher sollte bereits initial die Entscheidung für das zu wählende Therapieverfahren eine Risikokalkulation enthalten, die die zu erwartende Überlebenszeit und das erhöhte Risiko für vaskuläre Ereignisse nach Bestrahlung einbezieht.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
