Zusammenfassung

Die radikale Hysterektomie mit pelviner und ggf. paraaortaler Lymphonodektomie ist die operative Standardtherapie des frühen Zervixkarzinoms. Als operativer Zugangsweg hatte sich hier in den letzten 2 Dekaden die Laparoskopie zunehmend durchgesetzt. Frühe Beobachtungsstudien zeigten hierbei eine geringe Komplikationsrate gepaart mit scheinbar gleichwertigem onkologischem Outcome. Die jüngst veröffentlichte Laparoscopic-Approach-to-Cervical-Cancer(LACC)-Studie zeigte hingegen, dass die Rezidivrate bei minimal-invasiv operierten Patientinnen verglichen mit offen operierten Frauen 4‑fach und die Gesamtsterblichkeit 6‑fach höher war. In nachfolgenden Beobachtungsstudien wurden diese Ergebnisse in multiplen klinischen Settings überwiegend reproduziert. Die Gründe für das schlechte Abschneiden der Laparoskopie sind jedoch weiterhin unklar. Dennoch muss gegenwärtig in Anbetracht der vorliegenden Evidenz die Laparotomie als Standardzugangsweg angesehen werden.
