Sexualität und Kinderwunsch bei jüngeren Patientinnen nach Zervixkarzinom

Zusammenfassung

Stetig sich verbessernde Behandlungsoptionen und somit eine Zunahme von Patientinnen mit rezidivfreiem Langzeitüberleben nach der Diagnose Zervixkarzinom haben Auswirkungen auf die physische und psychische Lebensqualität nach erfolgreicher Behandlung („survivorship“). Sexualität hat einen hohen Stellenwert in Bezug auf die gesundheitsbezogene Lebensqualität insbesondere der jüngeren Generation, ebenso wie die Konsequenzen der Krebserkrankung und deren Behandlung hinsichtlich einer zukünftigen Elternschaft. Frauen unter 40 Jahren sind in einer Lebensphase, in der sie ihre sexuelle Identität entwickeln, ihre beruflichen Verpflichtungen oft anspruchsvoll sind und sie Erwartungen und Wünsche im Hinblick auf Partnerschaft und Familie haben. Dies alles hat mit der Krebsdiagnose und ihren physischen und psychischen Folgen möglicherweise ein jähes Ende. Mit der Zunahme des durchschnittlichen Gebäralters bei Müttern steigt die Anzahl betroffener Frauen mit dem Wunsch nach fertilitätserhaltender Therapie. Ein enges Zusammenarbeiten zwischen betreuendem Onkologen, dem Reproduktionsmediziner und dem Psychoonkologen ist Voraussetzung für einen adäquaten Beratungs- und Behandlungsprozess der jüngeren Patientin. Eine Schwangerschaft nach erfolgter Primärtherapie des Zervixkarzinoms ist möglich und scheint nicht mit einer ungünstigen Prognose für Mutter und Kind verbunden zu sein. Spezielle Fragestellungen, insbesondere die sexualmedizinische Beratung, die den Lebensabschnitt der jungen Zervixkarzinompatientin betreffen, stellen qualitativ hohe Anforderungen an die kommunikative und interaktive Kompetenz der behandelnden Ärzte. Die Teilnahme von onkologisch tätigen Ärzten und Ärztinnen an einem sexualmedizinischen Curriculum erscheint sinnvoll, wird aber nicht ausreichend in Anspruch genommen. Der Artikel beleuchtet einige grundsätzliche Aspekte im Umgang mit diesen sensiblen Themen und gibt Hilfestellung zur Gesprächsführung.
