Zusammenfassung

Die Zöliakie stellt eine entzündliche Dünndarmerkrankung mit deutlich steigender Prävalenz in genetisch disponierten Betroffenen dar (humanes Leukozytenantigen HLA-DQ2 und -DQ8). Ausgelöst wird die Erkrankung durch das Weizenkleberprotein Gluten. Das Autoantigen der Zöliakie ist die Gewebetransglutaminase (tTG2). Die Zöliakie kann in jedem Alter auftreten und sich sehr heterogen klinisch manifestieren. Es werden intestinale von extraintestinalen Symptomen unterschieden. In der Primärdiagnostik steht die Bestimmung der zöliakiespezifischen Autoantikörper gegen tTG2 mit hohen Sensitivitäten und Spezifitäten an erster Stelle. Die histologische Sicherung der Diagnose erfolgt durch den Nachweis von typischen Duodenalschleimhautläsionen. Der Eintritt einer Remission unter glutenfreier Diät wird definitionsgemäß gefordert. Die Therapie der Zöliakie besteht derzeit im Einhalten einer lebenslangen strikt glutenfreien Diät. Diese trägt dazu bei, Komplikationen durch die Malabsorption zu vermeiden.
