Nichtarteriitische anteriore ischämische Optikusneuropathie (nAION)

Zusammenfassung

Die nichtarteriitische anteriore ischämische Optikusneuropathie (nAION) ist die zweithäufigste Erkrankung, die zur Degeneration des Sehnervs führt. Die Pathogenese ist nicht geklärt. Vermutet wird eine transiente, nicht durch thromboembolische Ereignisse ausgelöste Ischämie im Bereich der kurzen posterioren Ziliararterien. Diagnostisch wegweisend ist die typische Anamnese eines plötzlich auftretenden Skotoms ohne assoziierte Schmerzen in Verbindung mit dem klinischen Befund einer ggf. sektoriellen Papillenschwellung, afferentem Pupillendefekt und zur Schwellung korrespondierendem Gesichtsfeldausfall. Dringendste diagnostische Maßnahme ist der Ausschluss einer arteriitischen AION. Gesicherte Behandlungsansätze gibt es nicht. Häufig angewandt, aber mit kontroverser Studienlage sind die Behandlung mit Steroiden und die Sekundärprophylaxe mit ASS (Acetylsalicylsäure). Ein Rezidiv am ipsi- oder kontralateralen Auge ist möglich.

Dies wurde in Einzelfällen für die arteriitische AION beschrieben.
