Aktueller Stand der endovaskulären Behandlung von Shuntdysfunktionen

Zusammenfassung

Shuntdysfunktionen umfassen Abnormalitäten des Gefäßzugangs, die eine adäquate Dialyse verhindern bzw. den Patienten gefährden. Die häufigste Dysfunktion betrifft den unzureichenden Shuntfluss (Flussdysfunktion). Sie beruht in aller Regel auf einer Stenose der am Shunt beteiligten Gefäße. Ihre Behandlung findet in den meisten Fällen durch ein endovaskuläres Verfahren statt. Shuntstenosen haben nach Dilatation eine hohe Neigung zu frühen Rezidiven. Die vorliegende Übersicht soll die Ergebnisse der endovaskulären Techniken darstellen, die zur Verbesserung der geringen Offenheitsraten einer einfachen Ballondilatation am Dialyseshunt Anwendung finden. Diese beinhalten die Applikation von Hochdruck‑, Cutting‑, Scoring- oder medikamentenfreisetzenden Ballons oder auch die Implantation von Stents oder Stentgrafts. Für die einzelnen Verfahren existiert eine sehr unterschiedliche Datenlage, die wiederum in sich eine hohe Heterogenität aufweist. Die Definition eindeutiger Empfehlungen ist deswegen nur für wenige Anwendungsbereiche möglich. Letztlich besteht auch weiterhin ein großer Bedarf der Evidenzverbesserung für diese für den Dialysepatienten sehr wichtige Therapieform.

Bei schwerer Aortenstenose und obstruktiver KHK empfehlen die Leitlinien derzeit eine chirurgische Kombi-Behandlung aus Klappenersatz plus Bypass-OP. Diese Empfehlung wird allerdings jetzt durch eine aktuelle Studie infrage gestellt – mit überraschender Deutlichkeit.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
