The 2010 World Health Organization (WHO) classification system for neuroendocrine neoplasms (NENs) defined all tumors with Ki-67 proliferation index > 20% or > 20 mitoses/10 high power fields as grade 3 (G3) neuroendocrine carcinomas (NECs).
1 However, it became evident that the differentiation state of the tumor was another very important factor impacting on patient outcome. Therefore, the 2017 WHO classification further divided the G3 category for pancreatic NENs into well-differentiated neuroendocrine tumors (G3 NETs) and poorly differentiated neuroendocrine carcinomas (G3 NECs).
2 In 2019, the WHO classification was updated to include other tumors of the digestive system.
3 A study comparing the WHO 2010 and 2017 classifications in patients with pancreatic NENs convincingly showed improved survival of patients with G3 NETs over G3 NECs, although both were significantly reduced relative to patients with grade 1 and 2 tumors.
