Bedeutung der Insulinresistenz beim polyzystischen Ovarsyndrom – prä- und postkonzeptionell

Zusammenfassung

Die Insulinresistenz (IR) stellt einen bedeutenden Faktor in der Pathogenese des PCOS dar. Sie scheint Mitverursacher der erhöhten Komplikationsraten in der Schwangerschaft zu sein. Der differenzierte Einsatz des Insulinsensitizers Metformin zur Ovulationsinduktion wird durch die Deutschen Gesellschaft für Gynäkologie und Geburtshilfe (DGGG) empfohlen. Eine Therapie in der Schwangerschaft zur Verbesserung des geburtshilflichen Outcomes bei Risikopatientinnen wird den aktuellen Leitlinien aufgrund fehlender Evidenzlage nicht empfohlen, erscheint jedoch unter kritischem Studium der verfügbaren Literatur vielversprechend.

Laser- und Lichtbehandlungen können bei Frauen mit polyzystischem Ovarialsyndrom (PCOS) den übermäßigen Haarwuchs verringern und das Wohlbefinden verbessern – bei alleiniger Anwendung oder in Kombination mit Medikamenten.

ICI-Therapie in der Schwangerschaft wird gut toleriert

Müssen sich Schwangere einer Krebstherapie unterziehen, rufen Immuncheckpointinhibitoren offenbar nicht mehr unerwünschte Wirkungen hervor als andere Mittel gegen Krebs.

Weniger postpartale Depressionen nach Esketamin-Einmalgabe

Bislang gibt es kein Medikament zur Prävention von Wochenbettdepressionen. Das Injektionsanästhetikum Esketamin könnte womöglich diese Lücke füllen.

Bei RSV-Impfung vor 60. Lebensjahr über Off-Label-Gebrauch aufklären!
