CRS/CRSwNP in 2050: a look into the future

Eleanor Roosevelt already quoted: "The future belongs to those who believe in the beauty of their dreams". We dream(ed) about understanding the pathogenesis of CRS to be able to give precision medicine to every patient that needed it. We now understand the importance of defining disease. Within the EPOS series, we defined many aspects of CRS, with and without nasal polyps, we proposed new categorization of CRS with emphasize on primary versus secondary disease, localized or diffuse and the sort of inflammation [

2]. We better defined how we evaluate the disease [
3]. We dream(ed) of new treatments that would control or even better cure the disease [
4]. We now have excellent treatment options tailoring to the severity of the disease, the impact on quality of life and the comorbidity [

6]. We proposed integrated care pathways to unite the treatment from the pharmacists, the GP and the secondary and tertiary specialist [
1]. We combined our clinics with allergologists, immunologists and pulmonologists to nominate treatable traits and co-morbidities and combine our approach for the best outcome for the patient [
7]. Many of the dreams that I had in 1990 when I entered the wonderful world of Rhinology have come true. But also, many are still to pursue. In this lecture we will discuss what we perceive as the most urgent needs of our patients, the big questions to be answered and the consequences for society. …
