Esophageal perforation is a rare but potentially life-threatening gastrointestinal emergency. The majority of cases are iatrogenic and have a mortality rate close to 10 % [
1]. Gastrointestinal involvement in sarcoidosis is uncommon, usually multifocal and often asymptomatic [

3]. Extrinsic esophageal compression by enlarged mediastinal lymph nodes has been previously reported as an uncommon mechanical cause of dysphagia in sarcoidosis [
