Ekzem — im Brennpunkt von Dermatologie und Allergologie

Zusammenfassung

Ekzeme gehören zu den häufigsten nicht übertragbaren entzündlichen Hauterkrankungen. Es handelt sich um eine nicht kontagiöse Epidermodermitis mit typischen klinischen (Rötung, Schuppung, Nässen, Krustenbildung, Hautverdickung, Lichenifikation mit Juckreiz) und dermatohistologischen (Akanthose, Parakeratose, Spongiose, lymphozytäre Infiltrate) Zeichen. Sie entstehen meist auf dem Boden einer Überempfindlichkeit. In einer vereinfachten Klassifikation kann man Ekzeme gliedern in Kontaktekzeme (irritativ-toxisch, allergisch), atopisches Ekzem (Neurodermitis), seborrhoisches Ekzem, nummuläres Ekzem und sonstige Ekzeme. Ebenfalls bewährt haben sich Einordnungen nach Ursache bzw. Auslöser (z. B. Nickelekzem, Chromatekzem) oder Lokalisation (Lidekzem, Unterschenkelekzem etc.). Den großen Fortschritten in der Erkennung pathophysiologischer Zusammenhänge der fehlgeleiteten Immunreaktionen steht die Problematik des Managements im Alltag gegenüber. Noch gibt es keine zielgerichtete Biologikatherapie, wie sie für die Behandlung der Psoriasis existiert, aber mit neuen Anti-Interleukin-4-Rezeptorantagonisten gibt es Hoffnung.
