Therapieaspekte interprothetischer Femurfrakturen – retrospektive Analyse von 70 Patienten

Zusammenfassung

Hintergrund

Interprothetische Femurfrakturen (IFF) sind seltene Verletzungen, die grundsätzlich osteosynthetisch oder endoprothetisch versorgt werden. Verschiedene Therapiealgorithmen wurden anhand sehr kleiner Studienkollektive vorgeschlagen. Einflussfaktoren auf das Outcome sind nicht bekannt.

Ziel der Arbeit

Ziel der retrospektiven monozentrischen Studie war es, anhand einer hohen Fallzahl Einflussfaktoren auf das Outcome zu identifizieren und einen Behandlungsalgorithmus abzuleiten.

Material und Methoden

Zwischen 2006 und 2020 wurden 70 IFF identifiziert. Es erfolgten 38 Osteosynthesen, 30 Revisionsendoprothesen und 2 Amputationen. Mit der Klassifikation und zeitlichen Zuführung zur Operation wurden 69 perioperative Variablen erhoben. Allgemeine und operative Komplikationen sowie Mortalität wurden im Nachuntersuchungszeitraum von einem Jahr festgestellt.

Ergebnisse

ASA- und Charlson-Score korrelierten mit der 1‑Jahres-Mortalität. Präoperativ erhöhte CRP-, erniedrigte Hb-Werte sowie der CHA₂DS₂-VASc-Score wurden als Einflussfaktoren auf die Mortalität identifiziert. Die operative Versorgung innerhalb von 24 h zeigte eine Tendenz zu weniger Allgemeinkomplikationen. Zuverlegte Patienten wiesen eine erhöhte Mortalität auf. Anhand der Klassifikation nach Pires et al. bzw. Füchtmeier et al. konnte keine eindeutige Therapieentscheidung getroffen werden. Relevante Kriterien für die operative Versorgung waren Frakturlokalisation, Implantatstabilität, Knochenvitalität, Verankerungsmöglichkeit der Revisionsendoprothese sowie Patientenzustand.

Diskussion

Die identifizierten Einflussfaktoren auf das Outcome entsprechen denen von Patienten mit hüftgelenknahen Femurfrakturen. Die Versorgung von IFF sollte zeitnah erfolgen. Anhand des bisher größten Patientenkollektivs wurde ein Behandlungspfad erarbeitet.
