Genetische Aspekte der erektilen Dysfunktion

Zusammenfassung

Hintergrund

Die erektile Dysfunktion (ED) ist eine häufige Erkrankung des Mannes, die die Lebensqualität des Mannes und seiner Partnerin einschränkt. Die Risikofaktoren zur Entwicklung einer erektilen Dysfunktion umfassen die Erkrankungen Diabetes mellitus, koronare Herzerkrankung und Hypertonus, aber auch Lebensgewohnheiten wie Rauchen, geringe körperliche Aktivität und Adipositas. Die vorliegende Arbeit fasst die aktuelle Literatur zur Genetik und erektilen Dysfunktion zusammen.

Material und Methoden

Es wurde eine Literaturrecherche mittels der Datenbank PubMed® zum Thema Genetik und erektile Dysfunktion durchgeführt und die relevanten Artikel ausgewählt.

Ergebnisse

Mehrere Aspekte zur Genetik bei ED werden in der aktuellen Literatur dargestellt. Ein Teil der Arbeiten beschäftigt sich mit der Assoziation von Kandidatengenen und dem Risiko zur Entwicklung einer ED im Vergleich zu gesunden Kontrollpatienten. Des Weiteren werden die Ergebnisse genomweiter Assoziationsuntersuchungen und als letzter Aspekt die Pharmakogenetik beim Ansprechen auf die Therapie mit Phosphodiesterase-5- (PDE-5-)Inhibitoren dargestellt.

Diskussion

Die heterogenen Daten bei genetischen Assoziationsuntersuchungen sind zum einen möglicherweise auf die kleinen Studiengruppen und zum anderen wahrscheinlich auf die verschiedenen untersuchten Ethnien zurückzuführen. Dies unterstreicht die Notwendigkeit der Überprüfung dieser Daten in großen multinationalen Studien.

Nun gibt es auch Resultate zum Gesamtüberleben: Eine adjuvante Pembrolizumab-Therapie konnte in einer Phase-3-Studie das Leben von Menschen mit Nierenzellkarzinom deutlich verlängern. Die Sterberate war im Vergleich zu Placebo um 38% geringer.
