Zusammenfassung

Die konfokale Laserscanmikroskopie (KLSM) ermöglicht die In-vivo-Diagnostik nichtmelanozytärer und melanozytärer Hauttumore mit hoher Sensitivität und Spezifität. Als optisches Verfahren mit sehr hoher Auflösung ermöglicht dieses Verfahren die Darstellung zellulärer Details in horizontalen Ebenen und wird daher auch als „optische Biopsie“ bezeichnet. In den letzten Jahren hat dieses innovative Verfahren Einzug in dermatologische Praxen genommen und wird täglich zur Hautkrebsfrüherkennung eingesetzt. Dabei wird die konfokale Laserscanmikroskopie als Zusatzdiagnostik bei klinisch und dermatoskopisch unklaren Läsionen verwendet. Aufgrund der zur Dermatoskopie deutlich höheren Spezifität kann durch die KLSM insbesondere die Anzahl unnötiger Biopsien verringert werden. Neben der Anwendung in der Hauttumordiagnostik kann die konfokale Mikroskopie auch zur Diagnostik bzw. Eingrenzung von entzündlichen Dermatosen eingesetzt werden. Zukünftig ist neben der in vivo Anwendung der konfokalen Mikroskopie auch die ex vivo konfokale Mikroskopie von Interesse, die in der mikrographisch kontrollierten Chirurgie eingesetzt werden kann und dieses Verfahren in Zukunft deutlich vereinfachen bzw. verkürzen könnte.
