Zusammenfassung

Die Koniotomie stellt einerseits die Endstrecke zur Sicherung des Atemwegs, in deren Verlauf weniger invasive Maßnahmen gescheitert sind, dar. Andererseits kann sie primär zur Etablierung eines sicheren Atemwegs erfolgen. Dieser ist zwingend notwendig, um den Patienten vor einer signifikanten Hypoxie zu bewahren. Die Situation stellt sich als „cannot ventilate – cannot oxygenate“ (CVCO) dar, mit der vermutlich alle in der Notfall‑, Intensivmedizin und Anästhesie tätigen Kolleg*innen schon einmal konfrontiert wurden. Evidenzbasierte Algorithmen zum Management des schwierigen Atemwegs und der CVCO-Situation sind etabliert. Scheitert die Oxygenierung mithilfe des Endotrachealtubus, der extraglottischen Atemwegshilfe oder Beutel-Maske-Beatmung, muss die Atemwegssicherung chirurgisch, d. h. mithilfe der Koniotomie, erfolgen. Prähospital beträgt die Prävalenz der CVCO-Situation ca. 1 %. Zur Frage nach der besten Methode liegen keine validen, prospektiven randomisierten In-vivo-Studien vor.
