Zusammenfassung
Im 19. Jahrhundert standen 3 verschiedene Methoden zur operativen Therapie von Blasensteinen konkurrierend nebeneinander, wobei sich die neuen Verfahren langsam gegen die alte Tradition durchsetzen sollten:

Seit dem Altertum wurde bereits der perineale Steinschnitt praktiziert, der in der Renaissance mit Einführung des Apparatus major durch Marianus Sanctus eine wesentliche Verbesserung erfuhr. Auch später wurde die Methode immer wieder durch neue Aspekte bereichert, bis sie schließlich von alternativen Verfahren mit deutlich niedrigerer Mortalität und Morbidität abgelöst wurde.

Der suprapubische Zugang war zwar ebenfalls seit der Antike bekannt, war jedoch in Mißkredit geraten, da man annahm, daß eine hohe Verletzung der Blase, die zumeist in offener Verbindung zum Knochen und dem Bauchraum stand, unweigerlich mit einem tödlichen Ausgang verbunden sein mußte. Der allmähliche Durchbruch der Sectio alta sollte somit erst sehr spät im 18. und 19. Jahrhundert erfolgen.

Als Errungenschaft des Erfindergeistes und der Gerätetechnik des frühen 19. Jahrhunderts galt dann der transurethrale Zugangsweg, der erstmals eine Steinsanierung ohne Schnittoperation ermöglichte und als Geburtsstunde der modernen Endourologie gelten darf.
