Zusammenfassung

Der Begriff pathologischen Fraktur wird in der wissenschaftlichen Literatur nicht einheitlich verwendet. Im Allgemeinen beschreibt er eine Fraktur, die ohne adäquates Trauma auf dem Boden einer fokalen Neoplasie des Knochens entstanden ist. Wichtig ist, dass eine pathologische Fraktur mitnichten auf dem Boden einer malignen Erkrankung entstanden sein muss, sondern auch eine Folge gutartiger Knochenveränderungen sein kann. Dennoch sind die malignen Erkrankungen, insbesondere die ossäre Metastase, eine wichtige, wenn nicht die wichtigste Differenzialdiagnose bei pathologischen Frakturen im Erwachsenenalter. Insbesondere im Hinblick auf eine mögliche maligne Genese stellt die klinische und bildgebende Aufarbeitung pathologischer Frakturen daher eine Herausforderung im klinischen Alltag dar. Dieses Kapitel vermittelt daher einen Überblick über mögliche Differenzialdiagnosen, den Workflow zur Aufarbeitung pathologischer Frakturen, bildgebende Differenzierungsmerkmale, Scoring-Systeme zur Abschätzung der Frakturgefährdung und mögliche Behandlungsansätze pathologischer Frakturen.
