Abstract
Benign anorectal conditions, such as abscess, fistulas and haemorrhoids may occur in the same patients, requiring wide excision of the diseased tissues. This may result in a large de-epithelized area of the anal canal, and removal of perianal skin, eventually leading to a stricture. A rectal mucosal advancement sutured to the apex of a skin rotation flap has been successfully used in four patients to partially reconstruct the anal canal and prevent the formation of an anal stenosis. Complex anal fistulas may be associated with haemorrhoids requiring a wide local excision. A large wound with loss of skin and epithelium may result, which is likely to head causing an anal stricture. After simple open haemorrhoidectomy, three muco-cutaneous bridges are usually left by the surgeon at the end of the operation to prevent a stenosis, but this may be impossible to perform following haemorrhoidectomy and fistulectomy, procedures requiring wider tissue excision. A technique to create a muco-cutaneous bridge despite an extensive surgical wound has been successfully adopted in four patients with long standing multiple anal fistulas, pelvirectal abscess and haemorrhoids.

Des lésions anorectales bénignes telles que abcès, fistule et hémorroïdes peuvent survenir chez le même patient, nécessitant des excisions tissulaires étendues. Ceci peut résulter en une large zone cruantée du canal anal ainsi que dans l'excision de la peau péri-anale favorisant éventuellement une sténose post-opératoire. Un avancement de la muqueuse rectale suturée à l'apex d'un lambeau de rotation cutané a été réalisé avec succès chez 4 patients afin de reconstruire partiellement le canal anal et prévenir la formation d'une sténose anale. Des fistules anales complexes peuvent être associées avec des hémorroïdes nécessitant de vastes excisions. La large plaie avec défect cutané et épithélial peut favoriser au cours de la guérison une sténose anale. Alors qu'une hémorroïdectomie simple, ouverte laisse en place trois pontsmu muco-cutanés. En fin d'intervention afin de prévenir une sténose, cette situation ne peut être réalisée lorsque l'hémorroïdectomie est associée à une fistulectomie qui nécessite une plus large excision. La technique de confection d'un pont muco-cutané malgré une large plaie chirurgicale a été réalisée avec succès chez quatre patients porteurs de multiples fistules anales, d'abcès pelvi-rectaux et d'hémorroïdes.
