Operationsziel
Schmerzfreiheit und Wiederherstellung der Schultergelenkfunktion.
Indikationen
Massenrupturen der Rotatorenmanschette beim jüngeren, körperlich aktiven Patienten.
Kontraindikationen
Omarthrose, Arthritis, Rotatorenmanschetten-Defektarthropathie, Lähmungen oder Funktionsstörungen des Musculus deltoideus.
Operationstechnik
Präparation eines distal gestielten Muskellappens aus dem anterolateralen Anteil des Musculus deltoideus auf eine Läge von 5 bis 6 cm. Akromioplastik und Resektion des lateralen Klavikulaendes. Ausgiebige Mobilisation der retrahierten Enden der restlichen Rotatorenmanschette, Tenotomie und Tenodese der langen Bizepssehne. Einschwenken des Muskellappens nach dorsal über den Defektbereich des Humeruskopfes und Vernähen bis in gesundes Gewebe der Restmanschette.
Weiterbehandlung
Anlegen einer Thorax-Arm-Abduktionsschiene für vier Wochen mit 60° Abduktion des Armes im Schultergelenk.
Geführte Krankengymnastik mit limitierter Flexion bis 90° und Abduktion bis 60° aus der Schiene heraus. Aktive Bewegungen frühestens nach der sechsten Woche.
Ergebnisse
Nach einem durchschnittlichen Zeitraum von zwei Jahren wurden alle 36 von Oktober 1992 bis November 1996 operierten Patienten nachuntersucht. Es handelte sich um vier Frauen und 32 Männer im Durchschnittsalter von 52,9 Jahren (39 bis 65 Jahre). Es zeigte sich eine deutliche Schmerzreduktion (siehe Tabelle 3) und eine Verbesserung der aktiven Schultergelenkfunktion. Der Constant Score betrug präoperativ 30,5 und postoperativ 66 Punkte. Als Frühkomplikationen fanden sich bei zwei Patienten Muskellappennekrosen, die eine Revisionsoperation erforderlich machten. Bei drei Patienten wurde im Verlauf eine Ruptur oder Insuffizienz des Deltoideus-Lappens diagnostiziert. Bei einem Patienten wurde aufgrund persistierender Beschwerden sekundär eine Schulterendoprothese implantiert. (Komplikationen von sieben Patienten siehe Tabelle 4.)
Summary
Objectives
Freedom of pain and restoration of shoulder function.
Indications
Massive tears of the rotator cuff in young physically active patients.
Contraindications
Glenohumeral osteoarthritis, cuff arthropathy, paralysis or functional disturbances of the deltoid muscle.
Surgical Technique
Preparation of a distally pedicled flap of the anterolateral part of the deltoid muscle over a length of 5 to 6 cm. Anterior acromioplasty and resection of the lateral end of the clavicle.
Extensive mobilization of the retracted ends of the remaining rotator cuff, tenotomy and tenodesis of the long head of the biceps. Rotation of the muscle flap in a posterior direction to cover the defect area over the humeral head. Suture covering well the healthy parts of the cuff.
Results
After an average of 2 years postoperatively all 36 patients (4 women, 32 men, average age 52.9 years [39 to 65 years]) operated on between October 1991 and November 1996 were examined. A marked reduction in pain (see Table 3) and an improvement of the active shoulder function could be obtained. Preoperatively, the Constant score amounted to 30.5 and postoperatively to 66 points. An early complication in form of a muscle flap necrosis occurred in 2 patients necessitating a revision. In 3 patients the postoperative course was marked by a rupture or insufficiency of the deltoid flap. Persisting symptoms in 1 patient necessitated a total shoulder replacement. The allover incidence of complications was 18.4% (7 patients, see Table 4).
