Luxation nach Ellenbogentotalendoprothese

Zusammenfassung

Hintergrund

Instabilitäten und Luxationen nach Ellenbogenarthroplastik, insbesondere nach ungekoppeltem Gelenkersatz, können Funktionsstörungen und Revisionseingriffe verursachen. Wir geben einen Literaturüberblick über Häufigkeiten und Ursachen dieser Komplikation und stellen eigene Ergebnisse mit der inkomplett gekoppelten GSB-III-Prothese vor.

Patienten und Methoden

Literaturangaben über Auslösemechanismus, Häufigkeit und Behandlungsstrategie der Gelenkinstabilität 5 international gebräuchlicher Prothesentypen wurden zusammengetragen. Eigene Komplikationen und Luxationsursachen bei 160 GSB-III-Prothesen wurden ausgewertet.

Ergebnisse

Ungekoppelte Oberflächenprothesen luxieren bei 0–15% der Fälle und machen durchschnittlich 25% aller Komplikationen aus. Betroffen waren alle Oberflächenprothesen und die GSB III, für die wir eine Entkopplungsrate von 7,4% ermittelten. Hauptursachen waren Rotationsfehler der Prothesenteile sowie Fehlpositionierungen des Rotationszentrums.

Schlussfolgerung

Die Ellenbogenendoprothetik stellt eine anspruchsvolle Operation dar, die noch immer mit einer vergleichsweise hohen Komplikationsrate verbunden ist. Die Luxationsrate kann bei richtiger Indikationsstellung, adäquater Wahl des verwendeten Prothesentyps und erfahrener Operationstechnik entscheidend gesenkt werden.
