Summary
Fistula clotting (16.5% of all patients) in hemodialysed patients (67 cases from 6 German centers) was found to be associated with a state of hypercoagulability characterized by increased platelet adhesiveness and increased thrombin production. Acetylsalicylic acid (Colfarit, Bayer-Leverkusen) in non-toxic doses normalized effectively increased platelet adhesiveness, diminished thrombin production and prevented fistula clotting in 4 patients with repeated thrombotic complications due to a hypercoagulable state.
Zusammenfassung
Bei 16,5% von 67 Patienten aus sechs deutschen Hämodialysezentren wurden thrombotische Verschlüsse der Cimino-Fistel beobachtet. Bei diesen Patienten bestand ein hypercoagulabler Zustand, gekennzeichnet durch gesteigerte Thrombocytenadhäsivität und gesteigerte Thrombinbildung. Acetylsalicylsäure (Colfarit, Bayer-Leverkusen) in nicht toxischen Dosen normalisierte die gesteigerte Plättchenadhäsivität und verminderte die Thrombinbildung. Bei 4 Patienten mit wiederholten thrombotischen Fistelverschlüssen wurden unter Colfarit postoperativ keine Fistelverschlüsse beobachtet. Die Substanz ist demnach möglicherweise geeignet zur Prophylaxe thrombotischer Fistelverschlüsse.
