Zusammenfassung
Hintergrund
Eine Konfrontationstherapie hat sich bei der Behandlung von Fear Avoidance Beliefs (FAB) als erfolgreich erwiesen. Zur Therapievorbereitung ist es erforderlich, eine Hierarchie der vom Patienten als gesundheitsschädlich eingestuften Aktivitäten zu erstellen. Zu diesem Zweck wurde die AMIKA-Skala, bestehend aus 50 Fotos, die ältere Menschen bei der Ausübung von ADLs zeigen, entwickelt und hinsichtlich seiner psychometrischen Qualität überprüft.
Methode
In einer Querschnittsstudie mit Messwiederholung nach 4 Wochen wurden neben der AMIKA Schmerzintensität und -häufigkeit, Funktionskapazität, FAB sowie körperliche Aktivität erhoben. Mittels Ultraschalltopometrie wurden zusätzlich Flexion und Extension der Lendenwirbelsäule gemessen.
Zum Messzeitpunkt t1 wurden 103 ältere Personen mit chronischem Rückenschmerz und einem Lebensalter von M = 71,41 (SD = 5,2) einbezogen. Zum Messzeitpunkt t2 wurden von diesen Probanden 67 erneut untersucht.
Ergebnisse
Die Retestreliabilität betrug r=0,63, die interne Konsistenz α=0,97. Zusammenhänge der AMIKA mit hoher Effektstärke zeigten sich für den Schmerz und die Funktionskapazität, mittlere Effektstärken für Zusammenhänge mit den konstruktverwandten FAB-Skalen. Keine Zusammenhänge zeigten sich für die objektiven Funktionsparameter sowie die berichtete körperliche Aktivität.
Schlussfolgerung
Die Ergebnisse berechtigen dazu, die Skala für die Erstellung einer FAB-Hierarchie im Rahmen der Konfrontationstherapie zu nutzen.
Abstract
Background
Confrontation therapy proved to be effective in the treatment of fear avoidance beliefs (FAB). Prior to treatment, it is necessary to establish a hierarchy of activities according to their perceived health hazards. For this purpose, we developed the AMIKA scale, which consists of 50 photos showing elderly individuals engaged in daily activities, and investigated its psychometric properties.
Methods
We conducted a cross-sectional study with repeated measurement after 4 weeks. At the time of both measurements, apart from AMIKA, we assessed pain, disability, FAB, and physical activity. Furthermore, we used ultrasound topometry for the assessment of flexion and extension of the lumbar spine.
At t1, a total of 103 elderly individuals with chronic low back pain participated in the study. Their average age was 71.41 years (SD = 5.2). Of these, 67 attended the t2 assessment.
Results
Retest reliability amounted to r=0.63 and internal consistency was α=0.97. Correlations of the AMIKA scores with pain and disability demonstrated large effect sizes, whereas effect sizes regarding other related FAB scales remained in the medium range. No correlations at all were found with respect to the objective ultrasound measurements and to self-reported physical activity.
Conclusions
The results allow the use of AMIKA as an instrument for the generation of a FAB hierarchy in the context of confrontation treatment.
