Zusammenfassung

Zwischen 8% und 16% der Weltbevölkerung leiden an einer chronischen Erkrankung der Niere (CKD). Hiermit gehen eine bedeutsame globale Krankheitslast und massive Behinderungsgrade einher. Die Prävalenz der psychischen Komorbidität ist epidemiologisch signifikant erhöht und mit negativen Verlaufskonsequenzen assoziiert. Präexistente affektive oder Stress-bezogene Störungen sind als Prädiktoren für ein CKD-Inzidenzrisiko bisher selten untersucht worden. Für eine „psycho-somatische“ und „somato-psychische“ Perspektive sind stets komplexe Interaktionen zwischen psychologischen und biologischen Ebenen ins Auge zu fassen. Eine Dialyse stellt im Endstadium die einzige lebenserhaltende Nierenersatz-Therapie neben einer Nierentransplantation dar. Sie erfordert höchste psychologische Anpassungsleistungen. Pathophysiologische Rückwirkungen eines chronischen Nierenversagens auf zentralnervös vermittelte affektive und kognitive Vulnerabilitätsrisiken sind eigenständig zu reflektieren. Resultate empirischer Studien zur Psychotherapie und Pharmakotherapie koexistenter affektiver und Stress-bezogener Symptome werden referiert.
