Orale Immuntherapie bei Nahrungsmittelallergien

Zusammenfassung

Die spezifische Immuntherapie (SIT) wird seit vielen Jahren erfolgreich in der Behandlung allergischer Erkrankungen, wie allergischer Rhinokonjunktivitis oder Insektengiftallergie, angewendet. Seit einiger Zeit wird auch verstärkt bezüglich kausaler Therapiemöglichkeiten, wie der SIT, für Nahrungsmittelallergien geforscht. Dabei steht momentan v. a. die orale Immuntherapie (OIT) im Fokus. Viele OIT-Studien bei Nahrungsmittelallergien konnten bereits Erfolge hinsichtlich einer Desensibilisierung zeigen. Ob auch langfristig eine orale Toleranz bei den Patienten induziert werden kann, ist bisher noch unklar. Erste Hinweise zu immunologischen Wirkmechanismen der OIT deuten an, dass auf T-Zell- und B-Zell-Ebene eine Immunmodulation stattzufinden scheint. Nebenwirkungen stellen jedoch bei einer Vielzahl der Patienten unter OIT ein Problem dar; deshalb kann diese Behandlungsform für Nahrungsmittelallergien zurzeit nicht außerhalb von klinischen Studien empfohlen werden. Somit bleibt nach wie vor das strikte Meiden des Allergens für Nahrungsmittelallergiker die einzige Option.
