A 45-year-old man with chronic hepatitis C developed facial hyperpigmentation 28 weeks after starting combination therapy with peginterferon-α-2a and ribavirin [dosages not stated]. A punch skin biopsy revealed chronic inflammation with alterations in pigmentation, as well as actinic keratosis. Immunohistochemical staining showed deposits of melanin in the pigmented macrophages and increased melanocytes along the basal layer of the epidermis. He was treated with 4% hydroquinone gel and a sunscreen, and his hyperpigmentation slowly improved over 12 months’ follow-up. …
