Personalisierte Tumortherapie bei gastrointestinalen Tumoren

Zusammenfassung

„Personalisierte“ Tumortherapie oder besser tumorgerichtete Therapien haben mittlerweile Einzug in die tägliche Praxis der Onkologie gehalten. In der gastrointestinalen (GI-)Onkologie haben sich diese Strategien in den letzten Jahren als Einzeltherapien und in Kombination mit Chemotherapien etabliert. Sie führen zu teils beachtlichen Verbesserungen hinsichtlich Tumoransprechen, progressionsfreiem und Gesamtüberleben. Durch das zunehmende Wissen über tumorrelevante Signalnetzwerke und die Möglichkeit deren gezielter medikamentöser Modifikation schreitet die Entwicklung auf diesem Gebiet schnell voran. Andererseits finden sich aber auch neue, bis dato unbekannte Arzneimittelwirkungen, die es einzuordnen und zu behandeln gilt. Daneben fehlen für die meisten dieser neuen Therapeutika (positiv-)prädiktive Biomarker, die einen wirklich rationalen Einsatz der Substanzen erlauben. Die vorliegende Übersicht beleuchtet den aktuellen Stand tumorgerichteter Strategien zu Diagnostik und Therapie von GI-Tumoren und leistet Hilfestellung dabei, die bisherigen Erkenntnisse in den täglichen klinischen Kontext einzuordnen.

Die Therapie von Echinokokkosen sollte immer in spezialisierten Zentren erfolgen. Eine symptomlose Echinokokkose kann – egal ob von Hunde- oder Fuchsbandwurm ausgelöst – konservativ erfolgen. Wenn eine Op. nötig ist, kann es sinnvoll sein, vorher Zysten zu leeren und zu desinfizieren.
