20. Schmerztherapie auf der (neonatologischen) Intensivstation

Zusammenfassung

Das nozizeptive System ist schon bei sehr kleinen Frühgeborenen funktionsfähig, wobei es enormen Entwicklungen unterliegt. Diese betreffen das Opioidsystem ebenso wie das schmerzhemmende System, welches im periaquäduktalen Grau seinen Ursprung hat. Pronozizeptive Systeme reifen eher als antinozizeptive, was Neugeborene sehr sensibel für schmerzhafte Reize macht. Es existiert keine spezifische Schmerzerfassung bei Neugeborenen. Beobachtungsinstrumente erfassen Schmerz ebenso wie Stress. Die beste Schmerztherapie ist die Schmerzvermeidung. Massiver unbehandelter Schmerz führt in der Neonatalphase zu einer Mortalitätssteigerung. Die Evidenz für einzelne nichtmedikamentöse und pharmakologische Schmerztherapien ist in der Neonatologie außerhalb des postoperativen Settings gering.
