Zusammenfassung

Hintergrund

Die Klavikulafraktur stellt nach der distalen Radiusfraktur die zweithäufigste Fraktur im Snowboarden dar. Diese sind immer noch eine Domäne der konservativen Therapie, bei Dislokation wird jedoch das operative Vorgehen empfohlen.

Methode

Als Osteosyntheseverfahren kommen dabei entweder konventionelle oder winkelstabile Plattensysteme oder aber intramedulläre Kraftträger in Frage. Intramedulläre Systeme konnten in klinischen Studien gleichwertige und teilweise bessere klinische Ergebnisse im Vergleich zur Plattenosteosynthese zeigen.

Ergebnisse

Laterale Klavikulafrakturen sind aufgrund der häufig vorliegenden zusätzlichen Verletzungen der Akromioklavikular- und Korakoklavikularbänder in Ihrer Behandlung komplexer. Die Hakenplatte als etabliertes Standardverfahren zeigt hier gute Ergebnisse bei gleichzeitiger Stabilisierung des Knochens und der Bänder, hat jedoch bezüglich der frühfunktionellen Nachbehandlung Nachteile.

Schlussfolgerung

Durch moderne kombinierte arthroskopisch-offene Verfahren mit Fadenanker-Flaschenzug-Systemen und kleinen winkelstabilen Platten kann eine frühere Mobilisierung mit sehr guten klinischen Ergebnissen erreicht werden.
