According to the Oxford Centre of Evidence-based Medicine (http://www.cebm.net/index.aspx?o=1025), methodologically well-conducted systematic reviews (known as meta-analyses when a quantitative analysis of results was carried out) represent the highest level within the hierarchy of scientific evidence. Particularly helpful are overviews of systematic reviews addressing a specific clinical question, especially those that focus on a diagnosis or therapy known for its provider-related heterogeneity. Without a doubt, such a clinical challenge can be found in the therapy of patients with myofascial pain of the masticatory muscles. This is obvious in the sheer variety and complexity of the therapeutic measures offered for patients with temporomandibular disorders (TMD). Many of us are familiar with the saying that how a TMD patient is treated has more to do with the floor on which the elevator in a dental clinic stops than with the evidence level of a therapy. …
