Zelluläre In-vitro-Diagnostik von Arzneimittelreaktionen

Zusammenfassung

Arzneimittel(AM)-Reaktionen können sich klinisch auf vielfältige Weise zeigen. Während die Mehrzahl auf dosisabhängige Nebenwirkungen zurückzuführen ist, stellt die Gruppe der unvorhersehbaren, immunologisch oder nicht immunologisch bedingten Unverträglichkeiten eine besondere diagnostische Herausforderung dar. Häufig fallen kutane Testverfahren negativ aus, Provokationstests sind zeitlich aufwendig und oft risikobehaftet. Vor diesem Hintergrund kommt zellulären In-vitro-Tests eine besondere Bedeutung bei der Identifikation des auslösenden AM zu. Während Basophilentests bei Immunglobulin-E(IgE)- sowie nicht-IgE-vermittelten Überempfindlichkeitsreaktionen eingesetzt werden können, dienen T-zelluläre Testverfahren der Diagnostik von AM-Exanthemen. Die Aussagekraft der einzelnen Tests kann von unterschiedlichen Parametern beeinflusst werden, wie dem der AM-Reaktion zugrunde liegenden Pathomechanismus, dem auslösenden Medikament oder dem Zeitpunkt der Testung. Neben der Kenntnis um die grundsätzlichen Prinzipien der einzelnen Assays ist daher auch das Wissen um diese zusätzlichen Faktoren unabdingbare Voraussetzung für eine korrekte Indikationsstellung und Interpretation dieser Testverfahren.
