Zytokinregulation und Schmerz

Zusammenfassung

Zytokine sind lösliche Peptide und Proteine, die im Organismus überwiegend von Immunzellen produziert und sezerniert werden. In zahlreichen Tiermodellen gehen eine endogene Vermehrung oder exogene Zufuhr proinflammatorischer Zytokine mit Schmerzverhalten einher. Andererseits kann dieses Schmerzverhalten durch Zytokininhibitoren in Modellen für entzündliche und neuropathische Schmerzen reduziert werden. Etliche Studien weisen auch auf die Bedeutung von Zytokinen bei unterschiedlichen schmerzhaften Erkrankungen beim Menschen hin. Bei verschiedenen chronischen Schmerzsyndromen findet sich systemisch ein proinflammatorisches Zytokinprofil assoziiert. Bei manchen Schmerzsyndromen ist dieses proinflammatorische Profil begleitet von einem Mangel an analgetisch wirksamen antiinflammatorischen Zytokinen. Eine Reihe von Fallberichten und offenen Studien, jedoch auch einige kontrollierte Studien zeigten Behandlungserfolge mit Zytokininhibitoren bei Schmerzen. Der folgende Artikel fasst die wesentlichen Befunde aus tierexperimenteller und klinischer Forschung zur Rolle von Zytokinen bei Schmerz und zur potenziellen Bedeutung zytokinmodulierender Substanzen in der Schmerztherapie zusammen.

Häusliche Gewalt in der orthopädischen Notaufnahme oft nicht erkannt

In der Notaufnahme wird die Chance, Opfer von häuslicher Gewalt zu identifizieren, von Orthopäden und Orthopädinnen offenbar zu wenig genutzt. Darauf deuten die Ergebnisse einer Fragebogenstudie an der Sahlgrenska-Universität in Schweden hin.

Fehlerkultur in der Medizin – Offenheit zählt!

Darüber reden und aus Fehlern lernen, sollte das Motto in der Medizin lauten. Und zwar nicht nur im Sinne der Patientensicherheit. Eine negative Fehlerkultur kann auch die Behandelnden ernsthaft krank machen, warnt Prof. Dr. Reinhard Strametz. Ein Plädoyer und ein Leitfaden für den offenen Umgang mit kritischen Ereignissen in Medizin und Pflege.
