Zusammenfassung
Der Mutter eines 13-jähriges Mädchen fällt zunehmend die ständige Müdigkeit und Unkonzentriertheit bei der Tochter auf; auch die Schulleistungen sind in der letzten Zeit schlechter geworden. Eine aktuelle Untersuchung zeigt ein leicht untergewichtiges Mädchen mit blasser Gesichtsfarbe und Mundwinkelrhagaden sowie folgende Blutwerte: Hb 9,8 g/dl, MCV 68 fl, MCH 23 pg, Serumferritin 5 μg/l, BSG 2/6. Die Patientin ist begeisterte Reiterin und ernährt sich ausschließlich vegetarisch. Die Menstruation hat vor 14 Monaten begonnen. Diagnose: manifeste Eisenmangelanämie bei erhöhtem Eisenbedarf im starken Körperwachstum und einer inadä quaten Ernährung. Therapie: orale Eisentherapie mit 50 mg Eisen(II)-Sulfat/Tag über 3–4 Monate, anschließend Kontrollen und Ernährungsumstellung oder ggf. weitere Therapie.
In den letzten Jahren ist deutlich geworden, dass ein schwerer Eisenmangel, insbesondere im wachsenden Organismus, nicht nur zu der bekannten Eisenmangelanämie, sondern auch zur Beeinträchtigung von bestimmten Stoffwechselfunktionen führen kann. So kann ein Eisendefizit offenbar die Gehirnentwicklung verzögern. Die Bedeutung des Eisenmangels, aber auch der Eisenüberladung im Kindesund Jugendlichenalter wird heute manchmal noch verkannt und in vielen Fällen relativ spät diagnostiziert.
