Unterschiede in der Anwendung der TMS im Kindes- bzw. Jugendalter im Vergleich zu Erwachsenen ergeben sich aus der funktionellen Unreife der schnell leitenden kortikospinalen Bahnen und assoziierter motorischer Systeme zum Zeitpunkt der Geburt. Die Axone der Pyramidenzellen sind etwa ab der 24. Schwangerschaftswoche bis ins Rückenmark deszendiert und bilden funktionelle Verbindungen mit dem spinalen α-Motoneuron aus (Eyre 2003). Bereits bei Frühgeborenen lässt sich die kortikospinale Bahn mit der TMS aktivieren (Eyre et al. 2000). Die zunehmend willkürliche und zielgerichtete Steuerung der Bewegung und damit der Muskulatur, wird mit zunehmender synaptischer Verknüpfung zwischen primär motorischem Kortex und anderen kortikalen Arealen, wie dem prämotorischen und somatosensorischen Kortex möglich. Ein Schwerpunkt der motorischen Entwicklungsdynamik liegt dabei in den ersten zwei Lebensjahren. Unter dem Einfluss der Reifung höherer kortikaler Funktionen entwickeln sich die motorischen Funktionen bis zum Adoleszenten- und jungen Erwachsenenalter.
