Die gepaarte TMS mit zwei überschwelligen Stimuli führt dann zu einer Veränderung der Amplitude des Test-MEP, wenn beide Stimuli mit einem Interstimulusintervall (ISI) von 10–200 ms mit derselben Spule über dem Motorkortex ausgelöst werden. Das Ergebnis der Interaktion zwischen den beiden Stimuli wird wesentlich durch das ISI und die Stimulusintensität bestimmt (Abb. 16.1). Bei ISI von 60–200 ms und deutlich überschwelligen Stimuli ist das MEP, ausgelöst durch den zweiten Stimulus, gehemmt. Diese Hemmung wird als intrakortikale Hemmung bei langen Interstimulusintervallen (engl. long-interval intracortical inhibition, LICI) bezeichnet. Bei kürzeren ISI von 10 ms bis 30–40 ms entsteht hingegen eine MEP-Fazilitierung, die entsprechend als intrakortikale Bahnung bei langen Interstimulusintervallen (engl. long-interval intracortical facilitation, LICF) bezeichnet wird (Claus et al. 1992; Valls-Solé et al. 1992).
