Bei der transkraniellen Magnetstimulation (TMS) wird über ein zeitlich veränderliches Magnetfeld im Hirngewebe ein elektrisches Feld induziert, um Nervenzellen zu reizen (Kap. 2). Hierfür wird in der Regel eine Reizspule tangential zur Schädeloberfläche aufgelegt und mit einem zeitveränderlichen Strom gespeist. Bei homogener Leitfähigkeit des Gewebes führt ein transkranieller Magnetimpuls zu einem Stromfluss im Gewebe, der parallel zum Strom in der Stimulationsspule verläuft und eine entgegengesetzte Stromrichtung hat (Kap. 2). Seit ihrer Einführung durch Barker et al. (1985) wurde die TMS in zahlreichen Untersuchungen weltweit eingesetzt, um die kortikale Erregbarkeit und Konnektivität insbesondere des motorischen Systems zu untersuchen oder um Hirnfunktionen passager zu beeinflussen (Kap. 1). Trotz der breiten Anwendung der TMS sind wichtige Fragen zur Ankopplung des induzierten elektrischen Feldes an die kortikalen Neurone bislang unbeantwortet:

Welche Anteile des Kortex werden präferenziell erregt?

Werden auch Axonbündel im subkortikalen Marklager von der TMS effektiv stimuliert?

Welche Neuronenpopulationen haben die niedrigste Reizschwelle?
-
An welcher Stelle werden die Neurone erregt?
Am besten sind die Mechanismen der transkraniellen Neurostimulation für die TMS des primären motorischen Kortex untersucht (Übersicht bei Amassian et al. 1987; Di Lazzaro et al. 2004). Im Folgenden wird der derzeitige neurophysiologische Kenntnisstand zur Wirkweise transkranieller elektrischer und magnetischer Stimuli unter besonderer Berücksichtigung der TMS des primären motorischen Kortex zusammengefasst.
