Zusammenfassung

Bei flachen Präkanzerosen kann der Laser in vielen Fällen erfolgreich eingesetzt werden, wobei die PDT eine zunehmend größere Rolle spielt. Der Einsatz des Lasers muss hierbei neu und klarer definiert werden. Er stellt eine Alternative zur PDT und zur Kryotherapie dar. Dickere bzw. knotige Läsionen sollten in den meisten Fällen wegen der Notwendigkeit einer histologischen Schnittrandkontrolle und besserer Langzeitergebnisse konventionell chirurgisch exzidiert werden. Der Einsatz von Lasersystemen bei malignen Tumoren stellt eine Einzelfallentscheidung dar und kann bei Inoperabilität oder in der Metastasenchirurgie indiziert sein. Bei geringsten Zweifeln an der Dignität eines Tumors muss eine Probebiopsie zum Ausschluss einer Malignität erfolgen. In allen Fällen ist beim Auftreten einer präkanzerösen Läsion oder eines malignen Tumors eine sorgfältige und engmaschige klinische Nachbeobachtung des Patienten erforderlich.
