Rationelle präoperative Diagnostik — Aus anästhesiologischer Sicht

Summary

Preoperative testing — from the anesthesiologist‘s point of view — should: (a) allow the estimation of perioperative risk of anesthesia and surgery, (b) identify medical problems relevant to anesthesia and (c) minimize the risk by optimizing peroperative therapy. Increasing economic constraints have led to a critical reevaluation of the cost-benefit ratio of preoperative testing in the last few years. It has become obvious that routine preoperative ‘screening’ tests (e.g. ECG, chest X-ray, lab) may be completely avoided in many patients with uneventful medical history and normal physical examination without increasing their perioperative risk. Furthermore, the patient’s exercise capacity, as well as the type or duration of surgery, have been clearly identified as independent predictors of perioperative (cardiac) risk. The following contribution summarizes current recommendations and algorithms regarding preoperative testing.

Zusammenfassung

Die präoperative Diagnostik verfolgt aus Sicht des Anästhesisten drei wesentliche Ziele: Sie soll dazu beitragen, (a) das Operations- und Anästhesierisikos abzuschätzen, (b) anästhesierelevante medizinische Probleme zu identifizieren und (c) das hierdurch entstehende perioperative Risiko durch Optimierung der präoperativen Therapie zu minimieren. Wachsende ökonomische Zwänge haben zur Folge, daß der zur Erreichung dieser Ziele erforderliche Aufwand an präoperativer Diagnostik in zunehmendem Maße einer kritischen Kosten-Nutzen-Analyse unterzogen wird. Dies hat in den vergangenen Jahren zu einer Abkehr vom System des ‘Routine-Screenings’ und hin zu einer individuell begründbaren, rationalen Diagnostik geführt. Im folgenden Beitrag werden die derzeit aktuellen Überlegungen, Empfehlungen und Algorithmen zur präoperativen Diagnostik aus anästhesiologischer Sicht dargestellt.
