Zusammenfassung
Bericht über sechs Patienten mit angeborenem Fehlen der Regenbogenhaut (Aniridie). In der Weltliteratur wurden bisher etwa 500 Fälle beschrieben. Die Häufigkeit der Aniridie liegt zwischen 1∶64000 und 1∶100 000. Die Mißbildung beruht auf einer Entwicklungsstörung des Ektoderms, die zeitlich in die 11–12. Woche der Embryonalentwicklung datiert wird. Die Vererbung erfolgt autosomal-dominant.
Die Penetranz des Gens ist auf 83,5±1,45% und die Mutationsrate auf 0,5·10−5 pro Gen und Generation errechnet worden. Es besteht keine geschlechtsdisposition. Als begleitende und den Irismangel verstärkende Störungen kommen am Auge vor: Amblyopie, Nystagmus, Glaukom, Bildungshemmung der Retina, Hornhauttrübungen, Trübungen und Lageanomalien der Linse, Entwicklungsstörungen der Netzhaut, vor allem der Macula lutea.
Die Prognose hinsichtlich einer Besserung des Sehvermögens ist sehr zurückhaltend zu beurteilen, da keine ursächliche Behandlung möglich ist. Um so größer ist die Bedeutung der Persönlichkeitsschulung möglichst innerhalb der Familie und später in speziellen Schulen für sehbehinderte Kinder.
Summary
Report on six patients with congenital absence of the iris.
500 cases are so far described in the world literature. The frequency of aniridia is supposed to be between 1∶64 000 and 1∶100 000. The malformation is due to a developmental defect of the ectoderm, which is dated to the 11th to 12th week of the embryonal development. The genetic transmission is autosomal-dominant.
The penetration of the gene has been calculated to 83,5±1,45% and the mutation rate to 0,5·10−5 per gene and generation. There is no sex disposition. Concomitantly inhibiting disturbances are seen: Amblyopie, nystagmus, glaucoma, developmental defects of the retina, corneal turbulences and topographic anomalies of the lense, and among the developmental defects of the retina especially those of the macula lutea.
The prognoses in regard to amelioration of the visus is to be judged very sceptically, since no causal treatment exists. This the more emphazises the care of the whole personality within the family and later on in special schools for visually disturbed children.
