Abstract
First description of femoroacetabular impingement goes back to the nineteenth century, although the pathomorphology itself has already shown in archaeological specimens 5000 years ago. The first description of the femoroacetabular impingement (FAI) concept in the literature was in 2003. FAI is therefore not a new pathology, but rather a conceptualization of a pre-arthritic pathomechanism that has existed for a long time. In the last 20 years, new insights mainly comprise the implementation of femoral torsion, the occurrence of extraarticular femoroacetabular conflicts, the three-dimensional understanding of the pathology, and specific knowledge on potential predictive factors influencing the clinical result. In particular, femoral torsion has been established as the third pillar of femoroacetabular impingement and directly influences the surgical treatment of this pathomorphology.
