Zusammenfassung
In diesem Kapitel werden neurobiologische und psychobiologische Faktoren von Chronifizierung und Neuroplastizität diskutiert. Chronischer Schmerz ist dadurch gekennzeichnet, dass es zu zentralnervösen Veränderungen kommt, die die Patienten für schmerzhafte, aber auch nicht schmerzhafte Reize empfänglicher machen und zu einer verstärkten Schmerzverarbeitung führen. Diese Spuren eines zentralen Schmerzgedächtnis ses können auf nicht deklarative Lernprozesse wie Sensibilisierung, operante und klassische Konditionierung oder Priming zurückgehen, jedoch spielen auch deklarative Lernprozesse wie das autobiografische Gedächtnis eine Rolle. Auch Modelllernen kann wichtige schmerzbezogene Gedächtnisspuren erzeugen oder vermindern. Affektive und kognitive Faktoren können zusätzlich Gedächtnisprozesse und Neuroplastizität modulieren. Dabei scheint bei Patienten mit chronischen Schmerzen eher die Extinktion schmerzbezogener Gedächtnisspuren als deren »Erlernen« gestört zu sein. Therapeutische Interventionen müssen deshalb das Erlernen schmerzinkompatibler Verhaltensweisen und den Abbau von Schmerzverhalten in den Mittelpunkt stellen, positive Erwartungen und Erfahrungen maximieren sowie verschiedene Umgebungen als Lernkontexte beachten und in das Training einbeziehen.
