Zusammenfassung

Ein gut geplanter operativer Zugang zur Revision einer Knieprothese ist die Grundlage für eine erfolgreiche Operation und ein funktionierendes Gelenk. Der Zugangsweg muss potenziell eine Erweiterung erlauben, damit die Komponentenentfernung der Primärprothese, das Débridement der Vernarbungen, die Balancierung der Ligamente, die Behandlung von Knochendefekten und schliesslich die Implantation der Revisionsprothese möglich sind. Das Risiko einer Hautnekrose kann durch die Verwendung von alten Narben und die direkte Präparation auf die Muskelfaszie verringert werden. Die Extension des Zugangs mit Entfernen aller Vernarbungen und die frühzeitige Erweiterung der Arthrotomie in den proximalen Quadrizeps oder nach distal durch eine Tuberositas-Osteotomie verhindern eine unkontrollierte Zerreißung des Extensorenapparats am Knie als Voraussetzung für eine gute postoperative Kniegelenkfunktion.
