Zusammenfassung

Zentrales Symptom des Schädel-Hirn-Traumas (SHT) ist die Bewusstseinseinschränkung, deren Einteilung sich an der Glasgow Coma Scale orientiert. Dieser Beitrag beschreibt neben der Pathophysiologie die Klinik und Diagnostik des SHT. Auch das therapeutische Vorgehen von der Primärversorgung bis zur intensivmedizinischen Behandlung sind hier dargestellt. Der primäre Hirnschaden nach SHT wird durch die Schwere des initialen Traumas determiniert. Sämtliche therapeutischen Bemühungen zielen auf die Minimierung eines sekundären Hirnschadens ab, der durch die vom primären Hirnschaden ausgelösten pathophysiologischen Prozesse entsteht. Für die Behandlung des SHT stehen konservativen und chirurgischen Optionen zur Verfügung. Der Prognose der betroffenen Patienten sowie der aktuellen Forschungslandschaft sind eigene Abschnitte gewidmet.
