Zusammenfassung
Die anaphylaktische Reaktion stellt eine gravierende, potenziell lebensbedrohliche Form der allergischen Reaktion dar. Als auslösendes Agens stehen Nahrungsmittel, Insektenstiche und Medikamente an vorderster Stelle. Die bedeutendsten Manifestationen erscheinen an Haut, Atemwegen, kardiovaskulärem System und Gastrointestinaltrakt. Führender Pathomechanismus ist die innerhalb von Sekunden bis Minuten eintretende Permeabilitätserhöhung der Kapillaren mit nachfolgender Plasmaexsudation und die zytokinvermittelte Vasodilatation. Entsprechend der Intensität der klinischen Symptomatik der Anaphylaxie hat sich eine Einteilung in Schweregrade von I bis IV nach Ring und Messemer bewährt. Entscheidend für die Diagnosestellung bei der akuten anaphylaktischen Reaktion ist die klinische Präsentation des Patienten mit Expositions- und eventueller Allergieanamnese und klinischer Untersuchung. Individuelle Risikofaktoren für eine schwere anaphylaktische Reaktion sind im Kindes- und Jugendalter ein Asthma bronchiale, eine anaphylaktische Reaktion in der Vorgeschichte, eine atopische Dermatitis mit hochgradiger Sensibilisierung und eine Mastozytose. Vor allem im Jugend- und Erwachsenenalter besteht ein Zusammenhang zwischen Augmentationsfaktoren und dem Auftreten einer anaphylaktischen Reaktion. Nach initialer Stabilisierung und Behandlung ist eine allergologische Diagnostik mit Bestimmung spezifischer IgE-Antikörper unabdingbar. Die initialen Maßnahmen haben zum Ziel, die weitere Zufuhr des Allergens zu stoppen, die Vitalfunktionen zu erhalten, die anaphylaktische Reaktion zu limitieren und schließlich zu beenden sowie den Übergang zur Diagnostik und zur präventiven Therapie zu schaffen. Die systemische Adrenalingabe stellt das Mittel der Wahl zur Kreislaufstabilisierung dar. Patienten, die eine schwere Anaphylaxie erlitten haben und bei denen ein weiteres Anaphylaxierisiko besteht, sollten mit einem Selbstmedikationsset ausgestattet werden und eine entsprechende Patientenschulung erhalten.
