Zusammenfassung
Bei den eosinophilen Pneumonien sind die Lungen durch eosinophile Granulozyten infiltriert. Meistens findet sich eine Bluteosinophilie und sehr konstant eine BAL-Eosinophilie. Die Klassifikation dieser heterogenen Krankheitsgruppe unterscheidet zwischen unbekannten Ursachen (idiopathische chronische eosinophile Pneumonie und akute eosinophile Pneumonie sowie eosinophile Pneumonien bei systemischen Syndromen wie eosinophile Granulomatose mit Polyangiitis (EGPA, vormals Churg-Strauss-Syndrom und hypereosinophiles Syndrom) und bekannten Ursachen (parasitär, infektiös, medikamenteninduziert, allergische bronchopulmonale Aspergillose). Die idiopathische chronische eosinophile Pneumonie spricht gut und rasch auf Kortikosteroide an, rezidiviert im Langzeitverlauf aber häufig. Die idiopathische akute eosinophile Pneumonie betrifft vorzugsweise junge zigarettenrauchernde Männer, sieht klinisch wie ein ARDS aus und spricht ebenfalls hervorragend auf Kortikosteroide an. Das hypereosinophile Syndrom wird nach Ausschluss bekannter Ursachen als konstante Bluteosinophilie länger als 6 Monate anhaltend mit Hinweisen auf einen Multiorganbefall definiert. Die myeloproliferative Variante spricht auf eine Therapie mit Imatinib an, während die lymphoproliferative Variante mit einem Anti-IL-5-Antikörper, z. B. mit Mepolizumab, behandelt werden kann.
