Zusammenfassung
Anatomisch ist das Koronoid als Region distal der knorpelfreien Zone der proximalen Ulna definiert. Die definitive Einschätzung der jeweiligen Fraktur erfolgt durch eine Computertomografie, möglichst mit 3D-Rekonstruktion. Koronoidfrakturen gehen in der Unfallsekunde fast immer mit einem Luxationsmechanismus des Ulnohumeralgelenks einher und sind von daher als Luxationsfrakturen einzuschätzen und zu behandeln. Neben dem betroffenen knöchernen Anteil des Koronoids spielen ligamentäre Zusatzverletzungen eine große Rolle: Je nach Luxationsmechanismus wird die Fraktur der betroffenen Region des Koronoids durch korrespondierende ligamentäre Verletzungen initiiert. Koronoidfrakturen werden nach O’Driscoll eingeteilt. Dabei werden sowohl die anatomische Lokalisation der Fraktur als auch der Verletzungsmechanismus berücksichtigt. Viele Koronoidfrakturen sollten operativ adressiert werden, in definierten Fällen kommt auch eine konservative Therapie in Frage.
