Summary
The authors report a case of bilateral stenosis of the supraclinoid carotid arteries in a young man. These stenoses are associated with poor cerebral vascularization, a pseudoangiomatous appearance of the base of the brain and intracavernous aneurysms of the internal carotid arteries. The similarity of the left and right carotid arteriograms obtained at a one year interval, refutes the possibility of arterial spasm due to subarachnoid hemorrhage. The evolution of the patient's condition with hemiparetic attacks which regressed, the young age of the patient and the aspect of collateral intracranial vascularization lead the authors to decide upon a diagnosis of Moyamoya disease as described by Japanese workers, using this term to define an angiographic syndrome rather than a particular clinical anatomical entity. The simultaneous existence of several aneurysms favours the congenital origin of the lesions observed .
Résumé
Les auteurs rapportent un cas de sténose bilatérale des carotides internes supra-clinoïdiennes chez un homme jeune. Ces sténoses sont associées à une pauvre vascularisation cérébrale, à une image pseudo-angiomateuse de la base du cerveau et à des anévrismes intracaverneux des artères carotides internes. La similitude des artériogrammes carotidiens gauches et droits pratiqués à une année d'intervalle réfute la possibilité de spasmes artériels dus à une hémorragie sous-arachnoïdienne. L'évolution par hémiparésis récidivants et regressivs, le jeune âge du patient et l'aspect de la circulation collatérale intra-crânienne permettent de conclure au «Moyamoya disease» décrit par les auteurs japonais. L'existence de plusieurs anévrismes s'inscrit en faveur de l'origine congénitale.
Zusammenfassung
Fallbericht mit einer bilateralen Stenose des supraclinoidalen Abschnitts der A. carotis interna. Diese Veränderungen waren begleitet von einer Rarifizierung der intracraniellen Gefäßabschnitte, pseudoangiomatösen Veränderungen im Basis-Bereich und ausgedehnten Aneurysmen im intracavernösen Carotisabschnitt beidseits.
