Summary
The osteogenetic capability of transplanted middle-ear ossicles was studied by means of (a) orthotoppc transplantation (i. e., into the tympanic cavity) or (b) heterotopic transplantation (i.e., subcutaneously into the lateral attic space). The present experiments in rabbits employed fresh homologous ossicles as well as Cialit-stored material. Heterotopic subcutaneous transplantations enables one to differentiate the osteogenetic capability of the ossicles themselves from that of the tympanic cavity. Two distinct phases were observed after the subcutaneous imbedding of a homologous ossicle. The first one, the osteoblastic phase, lasted for about ten days. During this time, osteoblastic cells proliferated and osteogenetis got under way in the grafted tissues. The live cells of the transplant died off on account of the antigen/antibody reaction. This in turn kept stimulating the production of new osteoblasts, initiating the second phase, which usually lasted for about three to six months. After this latter period, the transplanted ossicles were completely resorbed, presumably because their normal mechanical strain-sound conduction was not present.
The osteogenetic capability of the tympanic cavity was also assessed by means of the same two kinds of transplantation. Tissues from young calves, so called “Kieler” bone grafts, were being utilized. These experiments indicated that the tympanic cavity is an excellent bed for the receipt of osseous transplants.
Three factors are responsible for optimal integration of autologous and homologous ossicular grafts into the host tissues and for best preservation of the shapes of the ossicles: 1. transplantation into their normal site within the ossicular chain; 2. the action of a physiological, mechanical strain, and 3. the osteogenetic capabilities of both the ossicle and the tympanic cavity.
Zusammenfassung
Durch die orthotope (Paukenhöhle) und heterotope (Weichteilgewebe, seitliche Kuppelraumwand) Transplantation frischer and Cialit-konservierter allogener Gehörknöchelchen beim Kaninchen, konnte deren osteoinduktive Leistungsfähigkeit nachgewiesen werden. Auch die osteogenetische Potenz der Paukenhohle als knöchernes Ersatzlager konnte durch orthotope und heterotope Transplantation von xenogenem Knochenmaterial („Kieler Knochen”) nachgewiesen werden. Wie sehr die funktionelle, physiologische Verlagerung der Gehörknöchelchentransplantate in die Schalleitungskette auf die Formerhaltung und letztlich die taugliche knöcherne Integration in das Empfängergewebe Einfluß nimmt, ließ sick durch die heterotope Transplantation von allogenen Gehörknöchel-chen-Transplantaten aufzeigen. Wir ersehen daraus, daß die Voraussetzung für eine taugliche knocherne Integration eines allogenen and autogenen Gehörknöchelchen-transplantates die eigene osteoinduktive Fähigkeit des Transplantates, die starke knöcherne Ersatzleistung der Paukenhöhle Bowie die funktionsgerechte physiologische Verlagerung des Transplantates in die Schalleitungskette sind. Erst das Zusammenwirken dieser drei Faktoren sichert einen guten knöchernen Ein- und Umbau von allogenen and autogenen Gehörknöchelchen-Transplantaten in die Schalleitungskette.
