Langzeitheobachtungen operativ und konservativ behandelter Dupuytrenscher Kontrakturen

Zusammenfassung

Nach einer kritischen Darstellung der bisher in der Literatur vertretenen Theorien über die Ätiologie und Pathogenese der Dupuytrenschen Kontraktur wird über die Nachuntersuchungen von 48 Patienten nach operativer oder konservativer Behandlung berichtet. Zur Auswertung der Behandlungsergebnisse wird die Dupuytrensche Kontraktur in fünf Schweregrade eingeteilt. Bei der konservativen Behandlung wird besonders die Bedeutung und die Möglichkeit der Vitamin E-Behandlung dargestellt.

Die hierbei gewonnenen Ergebnisse lassen eindeutig den Vorrang des operativen Vorgehens bei der Behandlung der Dupuytrenschen Kontraktur erkennen, besonders bei mittleren und schweren Graden. Bei leichten Frühfällen jedoch hat, wie die Nachuntersuchungen zeigen, auch die konservative Behandlung durchaus ihre Berechtigung. Darüber hinaus sollte nach operativen Maßnahmen, um die Rezidivgefahr zu mindern, die Vitamin E-Medikation zu einem festen Bestandteil in der Nachbehandlung der Dupuytrenschen Kontraktur werden.

Summary

The current theories of etiology and pathogenesis of dupuytren's contracture of the hand are critically discussed. 48 patients with dupuytren's disease have been reviewed after a period of 2 to 21 years and the results of operative and conservative treatment have been examined. In the early stages of dupuytren's contracture vitamin E treatment is suggested a satisfactory effective medication in conservative grounds whereas the late stages require operative treatment. The possibilities of recurrence can be diminished by additional post-operative vitamin E treatment.

Résumé

Après un exposé critique des théories répandues dans les publications et relatives à l'étiologie et à la pathogénèse de la contracture de Dupuytren, on donne un rapport sur les examens subis par 48 patients sur la fin d'un traitement opératif ou conservatif. A propos du traitement conservatif l'attention est particulièrement attirée sur l'importance et les possibilités du traitement par la vitamine E.

Les résultats montrent nettement les avantages du procédé opératif dans le traitement de la contracture de Dupuytren, surtout aux stades moyens et avancés. Cependant, dans les débuts de la maladie le traitement conservatif est pleinement justifié, comme le prouvent les examens. En outre, pour réduire les risques de récidive après intervention chirurgicale, l'administration du vitamine E devrait être indiquée comme étant indispensable.
