Summary
The recording of His bundle potentials in patients with bundle branch block allows separate analysis of the conduction in the atrioventricular node and the rest of the His-Purkinje system. The mean value of the conduction time in the His-Purkinje system (HV-time) of 16 patients with a bundle branch block pattern in the ECG was 63 (40–100) ms. This value was significantly (p<0.05) higher than the mean HV-time (41 ms) of 13 patients with a normal QRS (30–50 ms). The intraventricular conduction time was more than 60 ms in 6 of 11 patients with a right bundle branch block and left anterior divisional block and in 4 of 5 patients with a left bundle branch block. The prolonged HV-time (>60 ms) in patients with bundle branch block suggests bilateral lesions in the His-Purkinje system.
Atrial pacing with increasing rates proved to be a useful aide in the functional analysis of the whole atrioventricular conduction system. Clinically latent intraventricular conduction defects were unmasked by the atrial pacing test. A second degree AV block, of type Mobitz II, with block distal to the His bundle was seen in 2 patients during atrial stimulation. In one of these patients the AV block, type Mobitz II, occurred simultaneously with an AV block, type Wenckebach, proximal to the recording site of the His bundle.
A pacemaker implantation was necessary in 7 of the 16 patients. Such implantation of an artificial pacemaker in patients with bundle branch block without the electrocardiographic documentation of a complete heart block is indicated if an additional delay of the HV-time or a block distal to the His bundle can be shown during atrial pacing. The significantly prolonged conduction time from the atrium to the His bundle in several patients with bundle branch block suggests an organic lesion in the AV nodal area in addition to the intraventricular conduction defects.
Zusammenfassung
Die Ableitung der Potentiale der Hisschen Brücke ermöglicht bei Patienten mit Schenkelblockbildern im Elektrokardiogramm eine getrennte Analyse der Erregungsleitung im Av-Knoten und im übrigen intraventriculären Leitungssystem. Der Mittelwert der Leitungszeit im His-Purkinje-System (HV-Zeit) von 16 Patienten mit einem Schenkelblock war mit 63 (40–100) ms signifikant (p<0,05) höher im Vergleich zu dem Mittelwert der HV-Zeit (41 ms) von 13 Patienten mit normaler Kammergruppe (30–50 ms). Bei 6 von 11 Patienten mit einem Rechtsschenkel-block und linkem vorderen Astblock sowie bei 4 von 5 Patienten mit einem Linksschenkelblock wurde eine über 60 ms hinaus verlängerte His-Ventrikel-Zeit festgestellt. Diese deutlich erhöhten HV-Zeitwerte (>60 ms) weisen bei Patienten mit einem Schenkelblock auf eine doppelseitige Schädigung des His-Tawara-Purkinje-Systems hin.
Bei der Funktionsanalyse des gesamten Av-Leitungs-systems hat sich die Frequenzbelastung mittels Vorhofstimulation als eine wertvolle Hilfe erwiesen. Klinisch latente, intraventriculäre Leitungsstörungen lassen sich durch die Frequenzbelastung demaskieren. So wurde unter Vorhofstimulation bei 2 Patienten ein Av-Block II. Grades, Typ Mobitz, mit einem Leitungsunterbruch distal der Hisschen Brücke beobachtet. Bei einem Patienten trat der Av-Block, Typ Mobitz, distal des His-Potentials simultan mit einem Av-Block, Typ Wenckebach, proximal des His-Potentials auf.
Eine Schrittmacherimplantation war bei 7 der 16 Patienten erforderlich. Sie ist auch ohne Nachweis eines kompletten Av-Blocks bei Patienten mit Schenkelblockbildern indiziert, falls unter Vorhofstimulation eine zusätzliche Verlängerung der HV-Zeit oder ein Block distal des His-Potentials nachgewiesen werden kann.
Bei mehreren Patienten mit einem Schenkelblock läßt die deutliche Verlängerung der Vorhof-His-Zeit vermuten, daß zusätzlich zu der intraventriculären Leitungsstörung eine organische Schädigung im Av-Knotenbereich vorliegt.
